Issuing certificates
The copy of the medical records can be obtained at the reception and booking desk.
The request can be made either at the time of discharge or later, by the discharged patient or his or her proxy, with his or her own and the patient’s identification.
The withdrawal is to be made Monday through Friday from 8:30 a.m. to 1:30 p.m..
Please be advised that for the issue of the copy there is a waiting time maximum of 30 days from the date of the request and a cost of € 30,00 including VAT. In case of urgency, it is possible, upon special request, to receive a copy of the medical records within five working days at a cost of € 50,00 including VAT.
The SANTABARBARA HOSPITAL, upon request, also provides for the shipment of the medical records at an additional cost of € 10.00.
I hospitalization certificates, with immediate delivery, can be requested from the doctor in charge.
